Here is a list of fees for private work carried out by Ashcroft Surgery. If you are a patient, please remember that this sort of work is not paid for by the NHS and therefore there is no requirement for GP practices to provide it. However, we at Ashcroft Surgery do provide it for the fees as stipulated below, which we feel are very reasonable (just compare it to what you have to pay a lawyer or a 5 minute diagnostic check at your local car garage).

- Certificates/1 page forms – simple non examination (i.e. private sick note, incapacity, bus pass pre retirement)………………….£20
- Reports/Forms WITHOUT examination (i.e. Gun Cert, Insurance claim (not DWP), Fitness to travel, TV Work/Acting, seatbelt exception, courts exemption, diabetic fitness)………………£35
- Report/forms WITH examination…………………………………..£65
(* if GP requests patient attendance still charge WITHOUT examination rate of £25) - Driving – PSV/HGV/Racing driver medical and report………..£140
- Power of Attorney/Mental capacity…………………………………….£65
Travel:
- Print out of current medications………………………………………£10

Access to Records
- Patient own record access …………………………………viewing free
- Copy of record – 35 pence per copy/printed sheet to max £10 if only computerized
- Copy of record, £50 max if mix of paper/computer record.
- Fees should be paid at time of request being made. Multiple cheques for amounts greater than cheque guarantee card value.
- Please note that other fees will be charged if there are requests for information from a 3rd party like a solicitor, the DVLA, social services etc.
