Gender Reassignment Registration

The process for gender reassignment registration is detailed below. This is the process which needs to be followed if a patient wishes to change details about their gender.  If there is incorrect information being held on the patient medical records regarding gender, please ensure you advise the GP surgery who will then arrange for any redaction to be completed via PCSE.
